Medieval: Total War to kontynuacja słynnej i wielokrotnie nagradzanej serii gier strategicznych, tworzonych przez zespół Creative Assembly. Po dalekowschodnich klimatach w Shogun: Total War przenosimy się w bardziej nam znane realia średniowiecznej Europy. Możemy kontrolować jedną z dwunastu najsilniejszych potęg militarnych istniejących w średniowiecznej europie. Gra łączy elementy militarne z handlem, dyplomacją oraz zarządzaniem surowcami. Przygotowano blisko sto różnych typów wojsk wyposażonych w autentyczną broń używaną w tamtych czasach (m.in. łuki, muszkiety, działa czy katapulty). Podobnie jak to miało miejsce w Shogun: Total War, tak i tu rozgrywka podzielona została na dwie części, tj. bitewną oraz tą, w której rządzimy całym swym królestwem. Bitwy rozgrywają w czasie rzeczywistym, w całkowicie trójwymiarowym otoczeniu (tereny pustynne, lasy, równiny, góry, itp.). Całość wykonana jest na ulepszonym engine z Shoguna, dzięki czemu mapy, na których toczą się starcia są nawet czterokrotnie większe niż to miało miejsce w Shogun: Total War, a na polu bitwy możemy zobaczyć nawet 10 tys. walczących jednostek. Część, w której rządzimy państwem rozgrywana jest w trybie turowym. Gracze mogą tu budować własne imperium, tworząc strategiczne alianse, ustalając trasy handlowe, odkrywając nowe wynalazki, budując zamki czy tworząc potęgę militarną zarówno lądową jak i morską.
